We enjoyed it very much every time we've been. The steak and shake is my favorite meal there but they have several options.

Yes, you need a ticket to get in. Reservations are the safer option, as it is a popular dining location. On slower days they are not necessary.

You can look up the menu online. Its not super cheap but the atmosphere is Worth every penny.

No, but I highly recommend Disney dining reservations. The wait can be excessive or they can be completely full.
